Preparing For Your Family Doctor Visit
- Please refrain from wearing scented products (due to patients and employees with allergies) and wear clothing that is easily removed if necessary.
- Wear loose comfortable clothing.
- You may also be asked to remove your clothing and wear a gown.
- Please remove items that may interfere with the exam.
- We are not responsible for lost or stolen items.
Appointment Information
- Appointments are required.
- Book your appointment online; Call our office at 416-461-9471 to book an appointment or come in person to book.
- Bring your valid Ontario Health card (or, if applicable, other provincial health card).
- If you do not have a valid Ontario Health card (or other provincial health card), please bring VISA, MasterCard or Debit Card as you will have to pay for the test. Quebec residents must pay for the test but a receipt will be issued.
- Please arrive 15 minutes prior to scheduled appointment time for registration.
- If you are late for more than half of your scheduled appointment, you may be re-booked to another date and your provider may charge you a fee.
- If you are unable to keep your appointment, please give at least 48 hours (2 business days) notification. .
